pick up location for the labs you order from TaylorMade wellness will be on the receipt. The labs are collection only.

RECEIPT MUST be presented to pick up your kit and you are required to leave your invoice at the medical clinic

Labs lists that are available to order from each company are listed. Otherwise you are responsible to order from the company or have Dr. Bean order the kit to your house. If you are ordering directly from the company or having the kit sent to you (Armin Labs) you will be paying the company directly.

Once you have ordered your kit from TaylorMade Wellness, you are responsible for shipping the kit back to the Laboratory Company. TaylorMade Wellness is not responsible for shipment.

**After you have made a purchase, Dr. Bean will email you with detailed instructions on how to collect
the sample and how to ship it
